Thank you for your interest in the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP). New centers wishing to participate in MBSAQIP must start by submitting the application form below. After submission the application will be reviewed by MBSAQIP staff. Once the application is approved, a MBSAQIP Program Coordinator will notify the applicant center's primary contact listed on the application and provide further instructions.

Submitting this application is an attestation of compliance with the MBSAQIP Standards. All new centers applying for accreditation must be fully compliant with all applicable MBSAQIP Standards at the time of application submission.
